ABR RESTRAINT FORM 

This is NOT the form you use to type into.
This is a crib sheet of instructions for filling out the report.
The form you actually fill out is called type_restraint.txt.
You can find it at: www.abrloggingandmining.com/forms/
or maybe on your desktop.

Use this form for all restraints.
This form must be submitted within 24 hours of the incident.

If this form cannot cover something about this restraint
then fill out a separate incident form and/or property damage
(those have their own forms).
If you do a second form, you can refer to information
that you already wrote on the first form so you don't have to repeat.

DON'T FORGET to select the appropriate Special Form web page
into which you will upload this form.

Wherever you see "TEXT_HERE" on this crib sheet
that is where you will type your information on the
real form.  If appropriate, you may write 'none' on the real form
the 'TEXT HERE' appears on this crib sheet.

Don't mess with the '.br' thingys.
You can put in blank lines but they will be removed
from the output.  They will cause a line break,
but the linebreaks you need are already supplied (by the '.br's).

DON'T FORGET to select "Restraint" as the 'Incident Type'
on the web page into which you will upload this form.
BE SURE to enter the minutes the restraint lasted on that page.
