This is a crib sheet of instructions for filling out the report
The form you actually fill out is called type_incident

This form must be submitted within 24 hours of the incident

DON'T FORGET to select the appropriate'Incident Type' circle
on the web page into which you will upload this form.

Wherever you see "TEXT_HERE" on the real form,
that is where you will type your information.

You can put in empty lines.  They won't show up
in the output but they will cause a line break.

Use this form for property damage
When you upload this form put it to the ATTENTION of Monte 
Osburn with the SUBJECT as the child who is
responsible for the damage
accompanied by an ABR work order
If more than one child was responsible for the damage,
you can do a ++ for that child or use the 'add subjects'
feature instead or generate an entirely separate note

(Until we start uploading work orders into the blog,
don't worry about putting the WO number in the box on the
web page; just enter it into the text of this form.)
