Crib Sheet for Incident Report (Other)

This is NOT the form you use to type into.
This is a crib sheet of instructions for filling out the report.
The form you actually fill out is called type_incident.txt.
You can find it at: www.abrloggingandmining.com/forms/
or maybe on your desktop.

This report is for all incidents (like AWOL and Injury)
that are not a restraint
or property damage (those 2 have their own  forms).
This form must be submitted within 24 hours of the incident.

DON'T FORGET to select the appropriate Incident Type on the web page
into which you will upload or paste this form.

Wherever you see "TEXT_HERE" on the real form,
that is where you will type your information.
If appropriate, you may write 'none' on the real form.

Don't mess with the '.br' thingys which stand alone on many lines.
You may put or leave blank lines in what you type.
The blank lines will be removed from the output,
but will cause a line break.

Then do a ++ note for each witness.
With the text: "Witnessed the incident"
Use the 'add subjects' feature to make this easier.
